Het tragische verlies van hun ongeboren kind heeft Kate (Vera Farmiga) en John (Peter Sarsgaard) enorm aangegrepen. Zowel hun huwelijk als Kate’s fragiele psyche heeft eronder te lijden, omdat ze wordt geteisterd door nachtmerries en demonen uit haar verleden. Het stel doet er alles aan om weer een normaal leven te krijgen en besluit een kind te adopteren. In het plaatselijke weeshuis voelen John en Kate zich op een vreemde manier aangetrokken tot een jong meisje met de naam Esther. Al gauw na thuiskomst vindt er echter een reeks gebeurtenissen plaats, waardoor Kate begint te geloven dat er iets mis is met Esther.
